There are many changes happening to our benefits system and Information Sheet 15 provides professional advice, guidance and support on how this will effect you and your patients.
Your patients may be anxious, upset or confused about what the changes may means for them and the sheets explains how you can help your patients apply for the appropriate benefits .
It outlines who you can go about providing evidence to support your patients' claims and how you need to suggest that it is important for them to collect evidence from all the professionals involved in their care in order to give them the greatest chance of making a successful claim.
Like all our Information Sheets this latest document aims to help raise awareness about and improve mental health services across the country.
Each of our sheets have been developed to provide valuable information, training and support tools on primary care mental health in Wales.
Their publication comes under the set objectives of our Gold Standard Project which has set the goal of providing good communication on mental health issues .
We have a range of other Information Sheets which are all available to download and view on and from our Information Sheets page.
If you have any questions about Information Sheet 15 or trouble viewing or downloading it please contact our Project Manager Lesley Hills via email at lhills@rcgp.org.uk or on 029 2050 4516.
The following is the final part of our Managing Dementia in Primary Care Training video series designed to support primary care teams in delivering the best quality care to patients with dementia and their carers.

Dementia is a hugely important topic as it's becoming more common in our communities so together with practice staff we need to develop services to tackle this problem.
The following video is based on feedback from patients and carers as they want ALL staff to understand dementia better because ALL staff have an impact on aspects of dementia care:
